INFORMATION ABOUT THE UNIVERSITY OF MINNESOTA’s (UMN) ANNUAL SILAS SURVEY

Payment Information:
* If you choose to be compensated for your participation, UMN will ask for your contact information (Name, Address, Telephone Number and Email Address) at the end of the survey. This information will only be used to process your payment and will be deleted after the completion of the study.

Registration, Privacy and Confidentiality Information:
* UMN requires an email address to issue invitations to participate and send up to two reminders to those who do not complete the survey. If you are worried that this may identify you, you can create an email address (Gmail, Hotmail, Yahoo) just for this study. All records of email addresses will be destroyed as soon as allowable under federal and university audit guidelines.
* UMN will not share, give, or sell your email address to any other organization. It will be used solely for our research.
* UMN will not secretly record or log any information about your visit. During data collection, UMN does collect IP addresses to ensure participants are valid. Email addresses are requested in case of problems with completing the survey. This information is deleted at the end of the study.
* UMN provides a secure, private, web server to protect against intermediate sites intercepting your data.
